Referral

Referral for Treatment

Under the Veterinary Surgeons Exemption Order (2015) a referral form is required to be completed by your vet prior to a Veterinary Physiotherapist treating that animal. This is in addition to the Veterinary Surgeons Act 1966 which details that animals should only be treated by persons qualified to do so. This applies specifically to a diagnosed medical condition. On this basis I are unable to treat your animal unless I have received a referral from your Veterinary Surgeon.

N.B. This does not apply to maintenance cases such as performance enhancement, as long as there are no underlying medical conditions present.

This is important as certain conditions and medications may render Veterinary Physiotherapy unsuitable.

By working closely with your Veterinary Surgeon we can ensure the best possible treatment is provided for your animal.

A veterinary referral is required for a physiotherapy assessment and treatment to take place on any patient that has a diagnosed condition, injury or illness. The referral form can be found and downloaded here which must be completed by the referring veterinary surgeon and returned prior to your session date.

Veterinary Referral

Before I can treat your animal I require a Veterinary Referral Form to be completed by your referring Veterinary Surgeon. Ideally this should be sent along with a full clinical history. Click on the button below to download the form and return to history@severnvp.co.uk , alternatively please provide me with your vets details and I can contact them on your behalf.
